No Purchase Necessary. No purchase or payment of any money is necessary to enter. You may
register for the drawing by visiting Arrowsmith Eye Institute the day of the drawing. Only one (I) entry
per person is allowed. Duplicate entries may disqualify an entrant at the discretion of Sponsor. Entries
must be completed and deposited in the designated drawing container at Arrowsmith Eye Institute
before the drawing to be held for which the entry is dated. Entries will not be returned and all information
written on the entries shall become the property of the Sponsor. By entering this contest entrants
hereby consent to Sponsor's use of all information on entry forms for marketing and other purposes.
All entries must be handwritten and must be complete and legible to be eligible. Mechanically
reproduced entries will not be accepted. Sponsor assumes no responsibility for lost, mutilated, late or
misdirected entries. Entrants need to be present to win, each entrant must sign his or her entry form.
Entries signed by proxy will not be eligible to win.
Eligibility. Contest is open to legal residents of the United States. Employees of Arrowsmith Eye
Institute and all their affiliates, subsidiaries, agents, retailers and immediate families of each and
persons living in the household of each are not eligible. Limit one prize per household, address or
family. All federal, state and local laws and regulations apply. Winners are subject to verification.
Prizes. One prize will be awarded for entries received by the day of the open house as expressed on the Arrowsmith Eye Institute website. The prize shall consist of
2 eyes for the price of 1, valued at $2674. Prize has no cash value. Winner will be determined by a
drawing to take place at Arrowsmith Eye Institute. Prize will be awarded, subject to the winner's
medical eligibility to receive the medical procedure. You must be present to win. No other discounts
will apply to the winner.
Odds of Winning. Odds of winning depend on the number of entries received. There must be 3 or
more medically eligible attendees present for the drawing to take place.
Terms and Conditions. No person under eighteen (18) shall be eligible to enter or to win. Winners
will be required to execute and return affidavits of eligibility and acceptance of the prize within
fourteen (14) days of notification. Procedure must be performed within 60 days of contest. By
participating in this contest, entrants agree to be bound by these official rules and by the decisions of
the Sponsor. By acceptance of prizes, winners consent to Sponsor's use of their name, and/or likeness
and biographical information for commercial purposes without additional notification or
compensation. Prizes are not transferable or assignable. If prize winner is not medically eligible to
receive the prize, Sponsor reserves the right to designate an alternate winner or not to award the prize.
Entrants agree to release, discharge and hold harmless Peter N. Arrowsmith, M.D., Peter N.
Arrowsmith, M.D., P.C., and Peter N. Arrowsmith, M.D., P.C., d/b/a The Arrowsmith Eye Institute,
their affiliates, parents, subsidiaries, advertising and promotional agencies, and their employees,
officers, and directors from any and all claims or damages, including personal injury and/or death
arising out of entrant's participation in the contest and in the acceptance of any of the prizes awarded
there from. Only one entry per person per year.
